The address to send completed background check is required.

* Payment Amount:
The payment amount is required.

The payment amount may not contain a credit card number.
Once payment has been completed, you must complete the form found at Request-for-Copy-of-Ohio-BCI-Check. The form can be mailed, faxed or emailed to BCI to complete your request.
